Baby Dedication

Baby Dedications take place on the last Sunday of every month. If you would like your Baby to be dedicated please fill out the following form and a member of our Team or Staff will be in contact with you concerning the next available date. This form must be submitted at least 2 weeks before the date of dedication otherwise it will be scheduled for the following month.
